You have the right to request access to the personal information we hold about you related to services you have received, with limited exceptions as permitted by law.
We work to ensure that the information we keep is accurate and up to date. If you believe information in your record is incorrect or incomplete, please contact us. We will review your request and make corrections when appropriate. If a change cannot be made, you may provide a written statement of disagreement that will be attached to your record.
